Doctor Tenma is a brilliant Japanese neurosurgeon who works at Eisler Memorial Hospital in Germany. He is doing well professionally and is even engaged to the director's daughter, Eva. One day, a boy with a gunshot wound to the head is brought to him. He is the older twin from a family of East German defectors. On the same day, the mayor collapses. In the end, Tenma chooses to treat the boy Johan before the mayor. His surgery is a success. However, other doctors operate on the mayor, and their surgery is not a success. Tenma is punished for disobeying orders. He is removed from his position, and Eva breaks off their engagement.

The director of the hospital and two of Tenma's superiors are found murdered. Johan and his twin sister Anna disappear from the hospital. Inspector Lunge of the BKA [German Federal Criminal Police Office] suspects that Tenma is the murderer.

Several years later, Tenma operates on Adolph Junkers, a key witness in a case involving the multiple homicides of middle-aged couples. Tenma hears Junkers say that a monster is coming. Junkers refuses to talk to BKA inspector Lunge, but he reveals to Tenma in fear that all his partners were killed by that monster. Junkers disappears from the hospital, and Tenma desperately chases after him. The incident calls to mind the disappearance of the twins and the murder of the hospital officials nine years ago. Nina Fortner is a student at the University of Heidelberg. She is an upbeat girl who works hard at her studies, club activities and part-time jobs. She also likes to imagine the "prince charming" who sends her romantic anonymous emails. Nina has no memories from before the age of ten. Meanwhile Tenma is after Johan, who has committed another murder right before his eyes. In Munich, he meets a blind old man who says that he taught foreign languages to Johan eight years ago.

Tenma uncovers evidence that reveals Johan will meet his twin sister Anna in Heidelberg when she turns 20. Tenma reveals everything to Maurer, a journalist for a local newspaper, and asks for his help in finding Anna. Maurer is unwilling at first, but he investigates with Tenma, and the two of them finally pinpoint Anna's location. Meanwhile, Anna has no memories of leaving the hospital. She was adopted by the Fortners and has grown up as Nina. On her 20th birthday, she receives a mysterious message inviting her to Heidelberg Castle.
