JD, Elliot and Turk spend their first night at the hospital working without a supervising physician, finding the responsibilities overwhelming. A car crashes into the hospital parking lot after a televised car chase.

Waiting on an answer from Carla, Turk spends time with his brother, who confesses he's getting a divorce. JD works with an old colleague of Dr Kelso, while Jordan and Dr Cox have a disagreement over finding out their baby's sex.

JD, desperate not to be seen as a geek by the other surgeons on rotation, tells them a secret about Turk, causing the two friends to fall out.

JD has to work on his day off and can't seem to leave the hospital. Turk looks for a heart donor for one of his patients. Carla is afraid that she wont have the courage to become a parent.

News of Ben's potentially terminal illness elicits different reactions in JD and Dr Cox. Turk discovers that he's considered the number two surgical intern, while Elliot's offered the chance to critique Dr Kelso's work.

Dr Cox starts to drink excessively after he makes a decision that leads to the death of three of his patients. Turk begins his rotation in orthopaedic surgery, but he isn't a fan of his attending doctor's touchy-feely methods.
