During Tokyo MER's establishment ceremony, the team gets a dispatch call to treat critical patients at a bus accident scene. Several of the patients are in mortal danger.

Tokyo MER is dispatched when a steel structure collapses at a construction site; resident doctor Hina Tsurumaki is against Chief Kota Kitami's decision to operate at the dangerous scene which risks the safety of the medical staff.

A criminal kidnaps a girl with a serious illness and takes her as a hostage; as bullets fly across downtown, Kota Kitami of Tokyo MER arrives at the scene and goes head-to-head with the police, who are reluctant to approach the captive.

Kota Kitami arrives at a tunnel cave in, only to find a doctor transporting a heart, needed for a transplant, buried under the debris. The tunnel collapses even more as vigorous rescue attempt continues.

The elevator, in which Kota Kitami's younger sister Suzuka Kitami and a pregnant woman are inside, suddenly stops due to a sudden fire. Naoto Otowa, and a prominent politician, Yugen Amanuma, are trapped along with them.

Eighteen school children suddenly go missing in the mountains. Kota Kitami arrives at the scene, and Tokyo MER members divide and execute their rescue efforts. The team members must now face new dangers alone.
