Tests reveal that Greene has a brain tumor, but he hides the news from Corday. Corday finds out that she is being sued for malpractice. She also reveals to Greene that she is pregnant. Abby's mother refuses to leave. Abby expresses resentment over her difficult childhood to her mother but resignedly brings her home for Thanksgiving dinner. Weaver shares an intimate dinner with a female psychologist from the hospital. The psychologist reveals that she is a lesbian.
A demented patient causes a shootout in the emergency room. After terrorizing people in the ER with a gun, she gets shot by a policeman. She soon dies. Benton later discover her dementia was caused by an aftereffect of his surgery that he and Cleo should have detected. Carter pursues a cute intern. After a particularly long surgery, Corday decides to take her maternity leave. And April Fool's pranks abound in the ER.
The detectives continue searching for a shooter who may be responsible for several unsolved murders years earlier.
Lewis clashes with Weaver over the treatment of an injured condemned murderer, Mike Kinney (CYRIL O'REILLY), who wants to die in the emergency room rather than wait for his execution. Abby urges her battered next-door neighbor, Joyce (recurring guest star CHRISTINA HENDRICKS), to seek shelter from Joyce's abusive husband, Brian (recurring guest star MATTHEW SETTLE). A weary Carter must endure the warring of his parents, Jack (recurring guest star MICHAEL GROSS) and Eleanor (recurring guest star MARY McDONNELL), as they plan their divorce. Carter also diagnoses an aspiring rapper who tests positive for HIV but refuses to tell his naïve girlfriend.
Benton takes a costly sign-language class. Romano suggests Benton enroll in a program whereby Benton would substitute for vacationing doctors at rural hospitals for high pay. As acting chiefof emergency services, Romano delegates a plethora of responsibilities to Greene and Weaver. Lucy and Carter share a romantic tryst while on duty. Greene treats hospital worker Mobalage for impotency. Joi's son dies of a drug pain overdose and the police blame Ross. Anspaugh closes Hathaway's clinic because Ross got the medical equipment that administered the fatal dosage from the clinic. Jeanie and Ross are involved in a vehicular accident on an icy road.
