Tom and Lynette learn more about their new neighbours, Bree answers a big question, and Betty loses something precious. Meanwhile, Gabrielle revisits her past when old friends come to town.

An outbreak of a sexually transmitted disease affects several members of the hospital staff, prompting Chief Webber to call a meeting. Meanwhile, the chief reluctantly confides to Dr. (Derek) Shepherd about his own medical concerns; Izzie and Cristina are worried by a patient's wife and daughter who are at odds over his care; Burke treats Bill, a college buddy whose medical diagnosis raises some questions regarding his wife's actions; and, just as Meredith and Derek grow more intimate, a bombshell is dropped.

As Meredith nervously awaits a final decision from Derek, her angst is interrupted when a train crash brings several seriously injured patients to the hospital, including Bonnie and Tom, a pair of passengers who've been impaled on a metal pole. And Addison Shepherd sees great potential in Izzie, who must decide whether her loyalty as Meredith's friend outweighs professional gain.

Cristina and Burke's wedding day arrives - along with the interns' first exam results - and a successor to the Chief is named, as Callie and George make a big decision about their relationship.

The truth is in short supply, as Meredith and Cristina lie to each other about the status of their relationships, George is given the unpleasant task of discharging an elderly patient who is no longer sick but refuses to leave the hospital; and Alex learns his fate after re-taking his medical exams.

The nurses at Seattle Grace Hospital go on strike, leaving the doctors to fend for themselves; Meredith's mistake in treating an elderly patient is brought to light by the patient's friends (June Lockhart, Betty Garrett and Rae Allen) and Izzie treats a pregnant teen.
