As JD is thrown into the chaotic world of hospital life, we not only get to see the quirky characters that he encounters, but also hear the humorous commentary of thoughts streaming through JD's head.

JD asks Dr Cox to give the new interns a pep talk, but is shocked to hear him tell them that they will mess up and kill a patient. Meanwhile, JD dwells on Dr Cox's revelation and wonders when he'll accidentally kill someone.
