Dr. Kelso (Ken Jenkins) assigns J.D. (Zach Braff) to work on a patient with Dr.Townshend (Dick Van Dyke Diagnosis Murder ) -- his longtime friend and colleague -- but things go downhill when Dr. Townshend insists on using his old school treatment method which proves to be an alien concept to an inept J.D. While Kelso defends his friend's technique Dr. Townshend's lack of current medical knowledge forces Kelso to admit his mistake. Meanwhile Carla (Judy Reyes) tells Turk (Donald Faison) she needs time to consider his proposal because she thinks he is still too immature prompting Turk to invite his married brother (D.L. Hughley The Hughleys ) for a visit to show Carla how seriously he is. In addition Elliot (Sarah Chalke) learns the sex of Jordan's (guest star Christa Miller Lawrence) baby but Dr. Cox (John C. McGinley) and his ex-wife can't agree if they want to know.

Dr. Cox (John C. McGinley) genuinely wants to make his relationship with Jordan (Christa Miller Lawrence) work but he struggles to keep her happy as she deals with the discomforts associated with her pregnancy. Meanwhile J.D. (Zach Braff) attempts to end his romantic dry spell by going on a date with a girl from the hospital gift shop and Carla (Judy Reyes) grows frustrated with Turk (Donald Faison) because he no longer romances her the way he used to. Also a very discouraged Elliot (Sarah Chalke) reluctantly takes JD up on his offer to move in with he and Turk after she is forced out of her apartment. Ken Jenkins and Neil Flynn also star.

J.D. (Zach Braff) and Elliot (Sarah Chalke) continue to seek relief from their problems by sleeping together but when things start to get a little out of control Elliot proposes that they become sex buddies which seems at first to be the perfect solution. Elliot thinks she has a handle on things but when Turk (Donald Faison) is forced to take the heat from Dr. Cox (John C. McGinley) for her mistakes with one of their patients he tries to lighten her load by suggesting that her patients switch to the care of other doctors. His good intentions put him at odds with both his friends and they are forced to reevaluate their arrangement . Meanwhile Dr. Kelso (Ken Jenkins) accuses Carla (Judy Reyes) of making her relationships with her patients the priority over their well-being. Neil Flynn also stars.

On J.D.'s last day at Sacred Heart Hospital he finally settles his feud with the Janitor tricks Dr. Cox into admitting their friendship and forecasts a happily-ever-after for the whole gang.
