An outbreak of a sexually transmitted disease affects several members of the hospital staff, prompting Chief Webber to call a meeting. Meanwhile, the chief reluctantly confides to Dr. (Derek) Shepherd about his own medical concerns; Izzie and Cristina are worried by a patient's wife and daughter who are at odds over his care; Burke treats Bill, a college buddy whose medical diagnosis raises some questions regarding his wife's actions; and, just as Meredith and Derek grow more intimate, a bombshell is dropped.

Derek and Meredith try (unsuccessfully) to be discreet as they spend the night together at Meredith's house, but their haggard appearance speaks volumes to everyone - including Bailey, who warns Meredith to end the relationship. Meanwhile George suspects an anesthesiologist of drinking on the job but is reluctant to speak up about it, Alex treats a former schoolmate who finds honor in self-mutilation, Izzie's boyfriend takes extreme measures in order for them to spend more time together, and Cristina seems to be battling a case of the flu.

When a family arrives at Seattle Grace Hospital after a serious car accident, Alex learns that their injuries are only the beginning of their problems. Meanwhile, as Dr. Webber recovers from brain surgery, it seems that his wife (guest starring Loretta Devine as Mrs. Webber) has some concerns about the state of their marriage.

The doctors of Seattle Grace help Izzie cope with the loss of her fiancé, Denny, and her decision to quit the internship program. Meredith must decide what the future holds after her hospital liaison with Derek. The Chief is confronted by his wife, Adele, after spending the night in his office, and George seeks relationship advice from a surprising source, on the season premiere of "Grey's Anatomy."

Richard goes into interrogation mode about a patient’s condition, Callie confronts George about his feelings for her, and Meredith and Derek meet about Doc.
