Meredith, Cristina, Izzie and Alex spend their first day as residents with their own interns, including George, repeating his internship after failing his exams, and Meredith's half-sister. Cristina searches for Burke, who is nowhere to be found and Richard resumes his position as chief of surgery.

Derek prepares to perform his first surgery since his breakdown; Owen has a PTSD episode which injures Cristina and puts their relationship in jeopardy.

Cristina tells Alex and Bailey about Izzie's condition; Mark is doing a face transplant on a patient and Owen and Alex are the donor gathering team; Bailey sends Callie after Derek to bring him back and Callie ends up staying in his vortex of self-pity.

The Seattle Grace staff continue to mourn George's death; George's mother arrives but is unsure of what to do with his organs.

Bailey's patient, Jackson, continues to get worse; Callie catches Owen in Cristina's bedroom; after a big blowout fight, Izzie is forced to face the truth about what is going on.

Izzie returns to work while the Seattle Grace residents fear possible layoffs; Bailey and Alex's difficult case is further complicated by her schizophrenic son.
