S15, E7: The 2009 crash of Cougar Flight 91 off the coast of Newfoundland stunned the aviation world, not simply because one of the most advanced helicopters in the world failed, but because one of the disaster’s passengers survived. We examine the doomed flight from St. John’s to an offshore drilling platform, and follow investigators as they piece together clues that ultimately reveal a fatal flaw in the state-of-the-art Sikorsky S-92’s design.
