Nurse Susan Manning (Jennifer O'Neill) is most vulnerable after the shattering suicide of her friend and employer. Filling out the 'routine forms' to apply for a new job at Dr. Quilly's renowned facility, she soon finds out that these are actually her own commitment papers. Just the beginning of a nightmare of confinement, delusion, and murder, Manning faces hellish treatment from workers and mentally unstable patients as she struggles to stay the single sane soul in this sea of insanity.
