When Frank's late-night buzz sawing and Marie's persistent meddling bother the neighborhood, they meet with Ray to discuss how he'll inform his parents of their complaints. When Frank and Marie show up unexpectedly at Ray's house, they assume that Ray planned the meeting to talk about them behind their backs.

As the elderly residents of the Malden Retirement Home relax after dinner, just down the hall, Miles Holling, who is the world's oldest man and just one day shy of his 115th birthday, is being suffocated by a shadowy figure. Captain Stottlemeyer's wife Karen (Glenne Headly), who once profiled Holling in a documentary film she made, suspects that the elderly man's death was no accident. But Captain Stottlemeyer, citing Holling's age, believes he likely died of natural causes. Monk is brought in to settle the dispute, and he agrees with Karen.

Kitty (Debra Jo Rupp) recruits Red (Kurtwood Smith) and the guys to help out at a church fund-raiser; Fez (Wilmer Valderrama) breaks up with Caroline (guest star Allison Munn). John Ratzenberger and Kevin McDonald guest star.

As J.D. (Zach Braff), Elliot (Sarah Chalke) and Turk (Donald Faison) end their first year as interns, they try to help an elderly man (guest star Don Perry) get an operation despite a lack of insurance, which draws the ire of Dr. Kelso (Ken Jenkins) in the season finale. Meanwhile, when Dr. Cox (John C. McGinley) and his ex-wife Jordan's (guest star Christa Miller, "The Drew Carey Show") relationship reaches a breaking point, she drops a bombshell on the gang. Judy Reyes also stars.

Brotherly love brings J.D. when his brother Dan comes back to town while Elliot and Carla begin moonlighting at a veterinary clinic; Tom Cavanagh Guest Stars. -- Dan decides to stay with J.D. for a while, since their mom's new fiancé kicked him out, but after spending some quality time with him at the hospital, Dan isn't sure if he likes the cynical person his little brother has become. Meanwhile, Elliot (Sarah Chalke) tries to deal with her own problems of having a long distance relationship and decides to try moonlighting as a pet examiner with Carla (Judy Reyes) to save up money to go visit her boyfriend as Carla saves up for her wedding.
