J.D. tries to persuade a smoker patient to quit smoking in order to impress Dr. Cox, who is just feeling lonely in his opinion. Elliot's attitude towards Carla annoys her, thus Turk is able to go on a date with Carla, when he convinces her to forgive Elliot.

A rival intern threatens J.D.'s ego and his standing in the eyes of the attending doctors; Dr. Cox takes an interest in Carla.

A hyped Dr. Cox (John C. McGinley) challenges the hospital staff not to lose a single patient before his shift ends at midnight while J.D. (Zach Braff) is enchanted by an unseen female patient (guest star Elizabeth Bogush, "Titans") who is trapped inside an MRI machine -- and even considers asking her out. Elsewhere, Turk (Donald Faison) and Carla (Judy Reyes) grapple with a pained patient (guest star Michael McDonald) as they try to re-set his dislocated shoulder. Sarah Chalke and Ken Jenkins star.

J.D. is frustrated when his budding romance with an ex-patient wilts as their dates are constantly interrupted by emergency calls, while the beastly Dr. Kelso becomes less fearsome to the wide-eyed interns, when Dr. Cox informs them how to avoid the blowhard's bite. In addition, a perplexed Carla and Turk deal with their first sexual intimacy complication.

When Dr. Kelso (Ken Jenkins) challenges the residents by offering a trip to a medical conference in Reno as a reward to the individual that presents the most interesting cast study, J.D. (Zach Braff) refuses to play along, earning him the respect of Dr. Cox (John C. McGinley). However, when Elliot (Sarah Chalke) confronts him about the competition, he realizes that it was in fact his fear of failure rather than his rebellious nature that kept him out of the running. Following some awkward moments and harsh words, Elliot and Carla (Judy Reyes) learn they have more in common than the just their relationships with J.D. and Turk (Donald Faison), bringing them closer together. In the meantime, Turk misses his opportunity to ask for a new laser on the one day a year that Dr. Kelso grants favors so he is forced to find another way to come up with the goods.
