After enduring year after year of botched Mother's Days, Frankie declares that the family gets to take a pass on doing anything at all for her this year. But after a man offers to buy Frankie's groceries at the market when she discovers that she forgot her wallet – and tells her to pay the kindly gesture forward – she decides to have the family pull each other's names out of a hat and do something nice for that person, which doesn't go as smoothly as planned.

The BAU investigates if bullying may be the link between two high school students who are the sole survivors after both of their families are killed.

Now attending Sacred Heart, J.D. has to deal with his own interns; Turk has doubts having children, then tries to prevent Carla from getting pregnant.

J.D. questions his relationship with his interns; Carla inadvertently gives Elliot an awkward nickname at her new job; Dr. Cox keeps Turk from warning Jordan about a con man.

J.D. is still dealing with his recent break-up and trying not to kill intern Keith, who has become annoyingly perfect. To make matters worse a favorite patient is in a serious coma and each of the staff finds their own way to deal with it.
