Dakota, Chelsea and Tanner explore the Old South Pittsburg Hospital in Tennessee; Dakota must prepare his sister, Chelsea, for a confrontation with a specter on the third floor that is said to attack only women.

The team revisits the St. Albans Sanatorium, a place so awful that Chelsea vowed she would never go back; considered to be the most haunted place on the East Coast, it is said to be filled with hundreds of entities, including a demonic force.

The team's road trip takes them to Lancaster, Ohio, to investigate a house of horrors; after Dakota instructs each member to explore the property all alone; Alex encounters something so evil it won't be believed if not caught on video.
