The Psychiatrist

S1, E6: Frank has lost his job as a fireman after continually missing the fire-engine. Convinced he is a failure, he decides to act upon Betty's suggestion to consult a psychiatrist. The therapist takes him back through his past, where his catalogue of disaster convinces the psychiatrist that Frank really is a failure and beyond help. Frank leaves the consultation a happy man, oblivious to warning shouts from a workman across the street. Seconds later he is buried under tons of gravel being dumped from the back of a truck.
