Carmela is joined at the hospital by family and friends. Meanwhile, Silvio briefs the crew on how he will handle things in Tony's absence; and Vito stirs things up by asking for some of the late Eugene's business.

Jennifer plans to relocate to St. Louis and take Rachel with her. Carter places Lucy in charge of the Halloween party at the dormitory so he can do research for his presentation. But trouble occurs when two medical students take drugs with alcohol and almost die. An operation reveals that a couple's little girl is genetically male. Corday, anxious to celebrate Halloween, gets a pleasant surprise from Benton, who dresses like the '70s film character John Shaft for a special date. Weaver gets disappointing news regarding her quest as Chief of Emergency Medicine.

Weaver visits a woman she thinks might be her biological mother. Later, Weaver realizes the woman cannot possibly be her mother. Hathaway finally makes an appointment to consult an obstetrician about her own pregnancy. Lucy begins her psychiatric rotation, but the patients don't take her seriously because she's so young. Greene helps Mobalage so he can testify at his own deportation hearing. Hathaway befriends a paranoid schizophrenic who delivers a normal baby.

Romano forces Benton to choose between two different fellowships. Greene and Corday become better friends on the way to a medical conference. Carter mentors a student. Lucy shows impressive skills during her psychiatric rotation. With amnesty granted, Mobalage now pursues a civil engineering license and leaves his job at the hospital. Jeanie treats a reverend who offers healing prayers in return. Hathaway's ultrasound is good.

Dr. Shaun Murphy questions the need for an elective surgery that will allow his young patient to smile for the first time. Meanwhile, Dr. Claire Brown and Dr. Morgan Reznick discover that their patient lied about her identity.
