When Cuddy’s handyman Alfredo, a young Latino and the sole provider for his family, falls from her roof and develops strange darkened pinkies, Cuddy joins the team in trying to figure out what’s wrong with him. Despite everything the team tries, the whole hand becomes infected. A trip to the young man’s neighborhood helps them understand Alfredo’s desire for a better life for his family, and provides a clue as to what might be causing his deadly illness.

As the FBI tries to prove that a corrupt construction company is to blame because they have been burying containers of toxic waste under school playgrounds, they discover that the students in the school have a higher rate of cancer and neurological problems. Charlie tries to mathematically prove cause and effect between the pavement materials and the cancer clusters. Meanwhile, Millie, Charlie's new boss at Cal Sci, orders him to spend less time helping out the FBI and more time in their department.

As Adam (Sean Giambrone) goes to great lengths to avoid swimming at school, jealousy prompts Barry (Troy Gentile) to undermine Erica’s (Hayley Orrantia) effort to make the world a better place.

Rebecca tries to get Justin to open up about his experiences in Iraq; Kevin deals with perceived homophobia at the law firm which may prevent him from becoming a partner.

Dr. David Morgenstern (William H. Macy) takes a trip down memory lane as he stops by County General and Sam's sister (Shannon Woodward) pays an unexpected visit.
